My message remains straightforward: public enthusiasm is advancing faster than scientific validation.
Public enthusiasm has outpaced scientific validation
Otherwise, optimization risks becoming a marketing slogan rather than a meaningful improvement in healthspan.
At the same time, it is evident that our field still needs to mature. More evidence, transparency. The distinction between science, promise, and marketing is blurry.
It also means paying attention to quality. Research associated with NUS Medicine and Chi Longevity co-founder Prof. Andrea B. Maier found that some mass-market anti-ageing supplements did not match their label claims, which is an important reminder that product quality matters just as much as the ingredient itself.
Healthcare systems globally urgently need a new generation of professionals trained in geroscience and precision geromedicine.
Building a new field is never straightforward. Healthy longevity medicine and precision geromedicine require new educational models, new collaborations, and new ways of thinking across disciplines.
Peptides act as signaling molecules, often intersecting directly with endocrine pathways. Mimicking physiology does not equal safety, particularly when dosing, purity, and interactions are poorly understood. The assumption that “natural” implies harmless is scientifically unfounded.
Across global longevity clinics, physicians report cases ranging from injection-site reactions to severe allergic responses, hormonal disruption, and early insulin resistance. Much of this is linked to self-experimentation: individuals sourcing unregulated compounds online and combining “stacks” without clinical oversight.
The current trajectory, DIY use in otherwise healthy individuals, is harmful for our field, let’s do not get ahead of the evidence.
But there is a risk: the field is moving fast, and with that comes noise. Without rigorous gerodiagnostics through validated biomarkers, and structured intervention pathways, “wellness” risks remaining superficial.
#Commercialisation is clearly outpacing clinical validation, warranting caution.
Indications remain insufficiently defined across metabolic, neurological, and broader ageing-related domains.
We need clinical development: trials are still positioned in phase II, with limited progression to definitive clinically relevant outcome studies.
At the same time, the market is accelerating rapidly. NAD precursors already represent a multi-billion segment, while intravenous NAD is widely used despite a huge lack of evidence and even a potential #safety issue.
For too long, health has been fragmented: gerodiagnostics in one place, training in another, outcomes nowhere clearly measured. That model does not hold.
I would be hugely cautious in every peptide, which is not FD approved, and do it only under medical guidance and um, under medical treatment.
Large-stage interventional trials remain limited. Endpoints are heterogeneous. Regulatory pathways are still catching up. Registries need to be deployed. Without shared diagnostic standards, translation will remain slow.
The gap is no longer biology, it is standardisation, trial design, and implementation into care!
Longer lives without maintained function are becoming unsustainable.
implementing Healthy Longevity Medicine, now coined Precision Geromedicine, is not always an easy path. Next to the tremendous joy, the journey is full of obstacles, doubts, and at times even allegations; all of which can make it sometimes difficult to see the huge progress our rapidly growing field is making to optimize health and healthspan of aging individuals.
Common challenges: - How do we integrate fast-moving science without overselling promise? - How do we harmonise standards of care across countries and clinical models? - How do we ensure rigorous data collection, scientific integrity, and meaningful outcomes for patients?
Precision Geromedicine is growing rapidly, fuelled by scientific breakthroughs including gerodiagnostics and gerotherapeutics. Yet one critical element is lagging behind: a structured, accredited pathway to train and certify the healthcare professionals who will deliver this care.
But measuring 249 biomarkers and broadcasting a psychedelic experience does not, by itself, generate actionable scientific insights.
If we want to bend the curve of aging, let’s ensure we do it with rigorou, rather than live-streamed intuition, which might induce replication at individual’s homes in potential unsafe situations.
Data without controls, ethics oversight, or reproducible methodology is #entertainment, not #evidence.
Without robust diagnostics to stratify risk, monitor target engagement and measure outcomes, even the most promising nutraceuticals risk under-delivering or being misapplied.
As Forbes journalist Sindhya Valloppillil highlighted, #GLP-1s and #peptides are now being sold like skincare: influencer-driven, cash-pay, and unanchored from proper oversight. The consumerization of longevity may fuel visibility, but without evidence and regulation, it risks undermining public trust.
The rise of celebrity-endorsed telehealth platforms selling GLP-1s, peptides, and 'longevity solutions' is turning medicine into marketing. The lines between evidence, aspiration, and commerce are blurring fast, and that should concern us all.
Right now, we are just exposing ourselves to physicians who are not properly trained. I can't exclude myself – I think I know what I'm doing, but I can't prove that I'm a good physician in this field because there's no one to certify me.
We would never accept this lack of education and certification for oncologists. We would never accept it for surgeons. But we are accepting it at this moment for anyone who calls themselves a 'longevity physician.' We have to do better.
The ABLE study, which I've presented at a few conferences, really shows that we can't just put longevity supplements on a shelf and let people decide for themselves whether to buy them.
The reason we need academic centers to stand up is that we currently have zero capacity to teach and train the healthcare professionals who should be in this field. How can we grow private clinics if we have no capacity in normal teaching hospitals for people to be trained? It's crazy.
Yet most corporate health efforts still sit at the level of benefits or stress management initiatives. Consider Healthy Longevity Medicine integration directly into the organizational design.
I think that very often clinicians do not have time and health coaches are very often not available.

I took for a, a time AKG alpha ketoglutarate. I but measuring first and then measuring again. I, I tried nicotinamide mononucleotide, so measured first NAD levels, then took it for three weeks. Then there was not an increase. I increased the dosage and measured again no. So which means I'm charged enough. I don't need it.
So I can tell you on Friday evening I will take many supplements because I'm a long flight. So we have times where we are exposed to changes, for example, time differences where. Absolutely. Supplements, if, if taken correctly can have a huge, huge impact.
I was blown away by their knowledge and enthousiasm and dedication to learn and experiment.
Our job is to make sure we inspire them before someone else tells them that science is too difficult.

We structured the open house for the public around three pillars: 1. The #science of aging biology: Making hallmarks of aging tangible and relating them to cardiometabolic risk, muscle strength, sleep architecture, inflammatory burden, and functional reserve. Heterogeneity is the rule: two individuals of the same chronological age can differ profoundly in biological resilience. 2. #Clinical precision in Healthy Longevity Medicine: Risk stratification, digital biomarkers, longitudinal tracking, and multimodal interventions are redefining medicine to keep individuals healthy(ier)! 3. #Translation into daily life: The image captured the essence: a crowded room full of participants engaging in beautifully guided breathing exercises by TSZ WAI NGAI. It illustrates a key principle: autonomic regulation is measurable, trainable physiology. Respiratory modulation influences our heart rate variability, stress reactivity, and inflammation.
Healthy longevity research cannot remain confined to #journals, #conferences, and clinical #trial units. Aging biology is modifiable and it therefore needs to be translated. Public engagement is not outreach; deep in my heart, it is #implementation.

we often have walking meeting. And we have that not only to expose ourselves to sunlight and going out of the offices, but also because we are training our brain quite differently because we have to walk, we have to talk, we have to look at where others are, and that's hugely important.
I think we have to bring it into the medical system measuring NED levels, and that's what I do in clinical practice. Seeing how much energy you need. Then subsidise that with the supplements, measuring it again, and also asking, do you feel better? Are you better? And that's, I think, proper care and proper use of supplements, uh, like NMN and NR.

We lit the first #candle at our first VIP network dinner at the NUS Academy for Healthy Longevity: fittingly hosted in our #gym at National University of Singapore, the space where we train not only #muscles but #mindsets.
The team was so happy that the biological age testing and knowledge sharing went smoothly (wonderful job team!!)! This is only the beginning!

The agreement was signed by Prof. George Panagiotis Chrousos and myself, with vital support from Dr. EVI HATZIANDREOU and Prof. J.J. (Hans) Meij, whose leadership in education brought this collaboration to life.
A new partnership has been formalised between the NUS Academy for Healthy Longevity at the NUS Yong Loo Lin School of Medicine and the Hellenic Institute for Healthy Longevity (#HIHL) through the signing of a Memorandum of Understanding (MOU). This agreement unites us to advancing active and healthy ageing through education, professional training, and public engagement.

As Director of the NUS Academy for Healthy Longevity, I see this integration of gerodiagnostics and gerotherapeutics as essential for strengthening the precision geromedicine framework that truly improves healthspan and lifespan.
The HELO initiative engages diverse communities worldwide to understand their perspectives on longevity, health, and wellbeing: because the public and potential consumers are the true drivers of this field.
Translating science into impact starts with listening, and this is exactly what HELO aims to do.

As someone whose work sits at the intersection of ageing biology, clinical translation and healthcare innovation, I emphasise the evidence based approach before jumping to market.
For gerotherapeutic innovation to succeed, the therapeutics should be linked to diagnostics.
We are moving from treating disease to building health #capital: at every age, in every profession.
We explored how #gerodiagnostics and #gerotherapeutics, the science of measuring and modifying biological ageing, are reshaping the future of #healthcare, #workforce design, and #leadership.
What inspires me most is the growing recognition that healthy longevity is no longer the domain of medicine alone. Executives, policy-makers, and innovators are realising that understanding ageing biology is essential for every organisation that wishes to remain future-ready.
The future of healthy longevity medicine depends on such cross-sector learning and it starts with curiosity, courage, and commitment to lifelong #education.
Together with J.J. (Hans) Meij and Guido KROEMER, we call for a global effort to integrate geroscience principles and healthspan education into everyday medical practice.
In our recent publication “Education in Healthy Longevity as a Prerequisite for a New Healthcare Model” in Geromedicine (Science Exploration Press), we outline how embedding healthy-longevity education across medical curricula, lifelong learning, and community engagement is essential to support longer and healthier lives.
We need to educate healthcare professionals and society at large.
Healthy longevity medicine should be built on data, ethics, and clinical validation, not on celebrity deals or marketing narratives.
I grew up in a family medicine clinic — quite literally at home, where both my parents practiced, always surrounded by patients. They never marketed. Later, while working in academic and teaching hospitals, we never had to market care either. We simply worked to #prove it, #improve it, and #deliver it to every patient. That is what medicine should be.
We argue that: • Medical, allied health and nursing curricula should include the biology of ageing, biomarkers of aging, and the principles of healthy longevity medicine. • Healthcare professionals need lifelong training to translate rapidly evolving longevity science into clinical care. • Public and community education are equally vital to empower individuals to manage their own ageing trajectories.
At the NUS Academy for Healthy Longevity (NUS Yong Loo Lin School of Medicine, National University of Singapore), this is exactly our focus. We provide evidence-based, CME-accredited courses designed to empower clinicians and professionals in the healthcare sector to apply healthy longevity science in clinical and public-health contexts.

We are including the average person, like you and me.
At the NUS Academy for Healthy Longevity we are building the evidence based foundation of precision geromedicine. We start with: 1. Meticulous evaluation of the literature; before any recommendation, we review all existing evidence systematically. 2. Clinical trials; we test interventions in diverse populations to understand efficacy and safety. 3. Precision geromedicine; only then do we tailor strategies to individual aging trajectories.
Healthy longevity interventions like spermidine hold great potential, but potential is not proof. At the Academy, we bridge that gap: transforming scientific promise into precision geromedicine.
I think that's the way we should go.
If it doesn't work, we have to discover why.
I love the human body, as well as those of animals and plants. I love creatures – everything that grows and has the capacity to rejuvenate. I think I am simply in love with the biology of aging. That's why I really want to maintain whatever has been built, understand why it's deteriorating, and what consequences that has.
I love making decisions, I love the knife, I love action. So, I think I'm a mixture of loving biology, making decisions, and being a bit of a politician.
I saw many appendectomies and colectomies, and I loved working with my hands in people's abdomens. I discovered that surgery is wonderful because you can touch the organs, and it's a sort of art. However, it's also very repetitive. I love to work with my hands, but not with that much repetition.
I was sort of forced to do Tai Chi every morning, but if you give me the opportunity, I'll choose kickboxing. I'm much better at that. So, it's absolutely not for me, but I do see its strengths – the balance, the relaxation component. It's quite rewarding. But I prefer kickboxing.
But the attractiveness of Singapore was its real dedication to making healthy longevity work. There's a dedication to change, to having a long-term impact, and to bringing evidence-based science and clinical care to the table. This is driven by the fact that Singapore will have a super-aged population in the coming years. There's a recognized need to change, and there's a long-term vision, which makes it very attractive.
Of course, it's also a hub in the APAC region. If you want to start public-private partnerships, it's much easier here than in Australia. Asia is the new hub for longevity investment, and Singapore is partly driving that.
The university is superb, great partnerships are already in place, and – most importantly – there is a driver for the entire field, which is the dean, Professor Chong Yap Seng. He is an unbelievable driver of this ecosystem. Twelve years ago, he was already thinking about what we now call healthy longevity medicine and how to make it happen.
He was revolutionary, studying children even before they were born to understand the early determinants of aging. This is all captured in the 'human potential' programs. Human potential applies to every age group, and I now use that term very often. We're talking about fulfilling human potential at every life stage.
Oh yes, the government is hugely invested. A couple of years ago, the government launched 'Healthier SG' [Healthier Singapore]. This program is embedded in the public health structure, where every Singaporean has a primary family physician. That GP is tasked with bringing preventative care into that individual's life. It's a huge initiative.
We are starting rigorous, science-based screening programs from the age of 40 onwards. But it's not just about screening; it's about helping families make lifestyle changes and focusing on the early detection and prevention of age-related diseases.
What we see now is a fusion of these ideas. You have a strong public health sector, we are discovering how to apply healthy longevity medicine in clinical practice, and now we're investigating how to merge the two. We are figuring out how to bring the academic thought of healthy longevity medicine into the public health sector. That's the stage we are at right now.
Healthy longevity medicine is the public-facing term. Precision geroscience medicine is the academic term we use in the specialized field. Healthy longevity medicine is defined as optimizing health and healthspan by targeting aging processes across the lifespan. Precision geroscience medicine is simply precision medicine for the aging field.
We've been operational for about four years and have already finished trials, but we didn't have the capacity to run parallel trials and truly grow the space. Now we do.
We have developed standardized operating procedures for measuring things like women's health, oral health, and cognition, especially for middle-aged individuals.
We have established biological, clinical, and digital biomarkers of aging, and we are matchmaking them with therapeutics in both unimodal and multimodal interventions.
We now have the capacity and infrastructure to study that.
I think we are in a space where we can allow ourselves to try and to make errors. We are toddlers in this field, not yet grown-ups. We don't know what really works yet.
So, we take a combined de-risked and high-risk approach.
Our trials always have primary outcomes that are known by the FDA.
We mix that conventional trial design with innovative elements.
For the first time, the primary outcome parameter in a trial like this was the change in those same epigenetic clocks.
We have the safety and recognition from the broader field of trialists, showing that we know how to randomize, we use the right software, and we apply tests properly.
Then, as the cherry on top, we add all our other biomarkers to get a better understanding of how these interventions act.
We are using the XPRIZE outcomes, which focus on immune, cognitive, and musculoskeletal function.
Alongside that, we are including many other parameters like proteomics, epigenetics, microbiome data, and digital biomarkers to track changes in health.
We only include individuals who are between the 25th and 75th percentile for these three functions.
Based on these three areas – cognition, immune function, and muscle – we build a personalized regimen that includes lifestyle interventions, dietary recommendations, nutraceuticals, and, in the next stage, drugs targeting the individual's specific gerotype.
We built grids and flowcharts to standardize this personalized approach.
It's a huge list of nutraceuticals, including urolithin A, NMN, and ergothioneine – compounds where we have at least some human evidence of a signal and a very low likelihood of harm.
When we finish this trial, which is running now and is very intense – it's like a boot camp for the participants – we won't be able to disentangle exactly what worked for any single person.
But we will know which combinations we prescribed based on their gerotype. Then, in the post-hoc analysis, we can see what was most likely to help.
We have a psychologist in the group who uses nudging techniques and helps us understand what people want to achieve.
We are combining diagnostics from psychology to biology to the social environment to give our intervention the highest chance of working.
However, this is how we think in clinical practice. This is the first time we are strategically bringing that mindset into a clinical trial design.
We do a lot of responder/non-responder analysis, and I think that's the key issue for our field.
We shouldn't underestimate how heterogeneous the aging process is.
Our AKG trial shows that we find significant epigenetic changes towards a lower biological age in individuals who were already biologically older, just by giving them one gram of calcium AKG for six months.
But we find the signal is especially strong in individuals who are more physically active.
Here you see that the combination of lifestyle and a supplement is very important.
We also see better results in certain demographics.
And we only applied it to individuals who were already epigenetically older – why would you charge your phone if it's already full?
It's the same for the NMN study we did a couple of years ago. After doing all the post-hoc analysis, we showed that the only individuals with a moderate NAD+ level increase showed clinical benefits from taking t
For me, it doesn't matter if a trial's primary outcome is positive or negative. The most important outcome is that we learn from the data.
We have many non-responders in these trials, but we also have an equal number of people who are huge responders. The challenge for us in the coming years is to understand this responsiveness—who are the responders? That will show us who should actually be taking certain interventions in clinical practice.
I have now found the overarching alliance of all patient organizations [Alliance of Patients Organizations Singapore – APOS]. We are now looking forward to building registries, giving good information to individuals, and recruiting from their members. It's a hugely active and proactive partnership. I am so happy that we finally have the consumer at the table, in addition to healthcare professionals, industry partners, governments, and policymakers.
Healthy longevity medicine should grow both publicly and privately.
However, what we really need is a very strong academic basis for it. If I could dream – and it seems the dream is coming true – we will have an alliance of academic clinics working together to build standards. We can test what works and what doesn't in a non-private environment, from implementation strategies to cost-effectiveness to running randomized controlled trials together.
We need to do the studies, but we also need teaching, teaching, and more teaching.
That's what we have to solve now. Otherwise, this can become a very dangerous field. There's a lot of 'snake oil' out there. We must establish a rigorous academic basis and have teaching hospitals that can offer rotations of six months, twelve months, or two years.

In the Clinic opening ceremony, the academy also formalised a strategic partnership with RMIT University , represented by Distinguished Prof. Professor Charlie C. XUE to accelerate research and education in Healthy Longevity Medicine.
Together, we will enable staff and student exchanges, joint PhD pathways, and collaborative clinical research between Singapore and Australia.

This week in Frankfurt, the Future of Nutrition & Longevity (FNL) Institute officially opened its doors, and I had the honour to deliver the keynote address.
What struck me throughout the evening was that nutrition in relation to longevity is no longer a topic for #science alone, it attracts global players such as Ferrero, Dr. Oetker, Danone and Unilever.
The entire food sector, from producers, processors, retailers to investors, is now starting to position itself as a #longevity #industry.
At FNL, the mission is clear: To #translate science, functional nutrition strategies into real-world impact for both producers and consumers.
Therewith, the institute is helping accelerate a shift from reactive medicine to proactive, nutrition-powered health.
A trillion-dollar opportunity for the global food sector: The healthy longevity movement is no longer confined to healthcare.
The global food industry is rapidly repositioning itself as a health and lifespan industry.
From ingredient manufacturers and consumer brands to agritech and supply-chain innovators, the sector is awakening to a new mandate: to deliver not just #calories, but biological #performance.

In our latest article, we confirm the feasibility of recruiting participants into geroprotective trials: The ABLE trial tested the feasibility of enrolling biologically older yet generally healthy middle-aged adults in Singapore for a gerotherapeutic intervention with alpha-ketoglutarate. Out of 467 individuals expressing interest, 120 were enrolled over 223 working days, with high eligibility (67.3% at screening) and a conversion rate of up to 48.4%. Most participants had a higher biological than chronological age, and recruitment was most effective through word-of-mouth: confirming the #feasibility of such studies.
Our mission: to optimize #health and thereby unlock the full potential of #life.
This is only possible with the best trial designs, strict adherence to #evidence generation, and by applying insights rapidly evolving from other medical specialties.
To generate evidence, we need to successfully recruit #participants into human clinical trials. We are meticulously evaluating each step to optimize our #research and eventually translate evidence into the #clinical practice of precision #geromedicine.
